RN Care Management offers skilled care and services in your home after a stay in a hospital, rehabilitation facility or after an acute illness.
The RN Care Manager will conduct an assessment and craft an individualized care plan that may include the following services:
- Manage medication, refill pill boxes, pharmacy requirements including pick-up of prescriptions
- Ensure you or your loved one is receiving and maintaining adequate nutrition and hydration
- Accompany you or your loved to physician / medical appointments and advocate on your behalf
- Chronic disease management (diabetes, congestive heart failure, COPD, hypertension, etc.)
- Periodic memory screenings
- Provide information to and coordinate treatments with physicians and other health care providers
- Provide information to family, POA, or other responsible persons via telephone or email
- Arrange for additional home care services as identified and requested
Surrey’s Care Managers provide personal advocacy and support for you and your family. Our care managers can accompany you to the emergency room or medical appointments. Because of their vast experience they know the necessary questions to ask and are able to make sure you fully understand what care you are receiving and why it is or isn’t necessary. They will help you understand any procedures you may need and explain test results. Importantly, Surrey Care Managers support the families they work with emotionally and act as a liaison communicating with family members. Care Managers will also advocate for you in determining if you qualify for any county and veterans benefits and will work with long term insurance companies.
